How to request a record

  • Complete the prescribed request form (Form 2 under the PAIA Regulations), available from the Information Regulator’s website or from us.
  • Send it to our Information Officer at instruments@essilor.co.za or deliver it to our office.
  • Give enough detail to identify the record, state the right you want to exercise or protect, and how you want to receive the record.
  • Pay the prescribed request fee and, if access is granted, any access fee. We will tell you the amount.
  • We will respond within 30 days. This may be extended once by up to 30 days, as PAIA allows.
